Official Transcript & Diploma Request Form

This form is especially for Grads who need a final official transcript sent to a school or institution and/or need to get hardcopies mailed to you to save for later use. Grads may also request a diploma certificate from SC TOP.

**If your college/university will only accept official transcript via a secure 3rd-Party platform (not email), such as Naviance or Parchment, SC TOP does have that option for our graduates. There will be a separate request form and added fee for that delivery service.

Please submit a clear scan of the parent signature on the final approved transcript or on a blank piece of paper (no lines).

Official Transcripts--YOUR Backup copies
We can mail 2 official transcript copies for you to keep on file, should you ever need them later on. State your mailing address so we can mail that to you.
- Home Address: Same as listed on the Transcript
- Other Personal Address: Different from what's listed on the Transcript
Personal Mailing Address
PO Box or family/relative with reliable delivery that is different than the home address listed on the student transcript

What school/institution do you want us to send transcripts on your behalf? We have email, fax or snail mail--include the specific contact person and preferred contact. ***NOTE: Parchment delivery is a separate request form and an additional $10 fee for each request.
Do you need a diploma?
Indicate if you need a diploma also mailed to your home address along with the transcript copies. **For those who did not participate in the graduation program.
