This form is especially for Grads who need a final official transcript sent to a school or institution and/or need to get hardcopies mailed to you to save for later use. Grads may also request a diploma certificate from SC TOP.

What school/institution do you want us to send transcripts on your behalf? We have email, fax or snail mail--include the specific contact person and preferred contact.
Official Transcripts--Backup copies
We can mail 2 official transcript copies for you to keep on file, should you ever need them later on. State your mailing address so we can mail that to you.
Do you need a diploma?
Indicate if you need a diploma also mailed to your home address along with the transcript copies.
